Peabody, Massachusetts - Aviv Centers for Living relocated their existing skilled nursing operation from Swampscott to a new building located on their existing Woodbridge Assisted Living complex in Peabody, Massachusetts. The new resident-centered household model of healthcare merged Aviv’s skilled nursing and senior housing for the people of the North Shore into one location. The center offers a variety of thoughtful outdoor spaces and amenities coupled with comfortable interior public spaces for dining, socialization, and programming. The main floor is an active area where residents can enjoy coffee at the Bistro, visit the salon or gift shop, and attend musical events in the Great Room. The Adult Day Health Care and an inter-generational Early Childhood Program increase the activity within the center and its interaction with the greater community.

The Center consists of 144 beds on four floors. Each of the three upper floors is comprised of three households of 16 beds, which are attached to a central core of service, support, and administrative space. Each household has 12 private rooms and 2 semi-private rooms, with a private shower in each room. Each of the four floors has a different primary focus. The fourth floor is dedicated to long-term care. The third floor is dedicated to short term rehabilitation and post-acute services organized around a large gym. The second floor is dedicated to memory support and offers easy access to secure outdoor gardens.

The new building is situated on a sloped site, creating a four-story front with a three-story rear exposure. The main entry, courtyards, and Adult Day Health and Early Childhood Program each have outdoor gardens and activity areas. Memory support residents can access secure gardens directly from their households. A one-story link connects the new building to the existing assisted living facility and creates a common service and receiving area that has direct access to the center’s kitchen.
